Time for a change

In this week’s article, ‘Reliability Moderneering: Time for a change – Acquiring safety through reliability’, by Kelly Callier, Certified Reliability Engineer, Callier recalls a life-changing event that changed the course of his professional career.
“It had been a normal day at the refinery, until I witnessed the single most life-changing event that rocked my professional world. After that day, I was never the same professionally. I decided I had to make a difference. I needed to make a difference. I was going to make a difference,” Callier writes. “I personally witnessed a gentleman, a father of two boys, a devoted husband, a coach, and my friend, brutally have his face disfigured, which caused instant death. The root cause of such an accident was a bull plug located on a chemical injection system, which had badly corroded the threads internally, that the plug was unable to withstand the pressure of the system.”
